I want to connect 2 monitors to my computer as well as output t my tv via HDMI . What video cards will do this?

What graphic board do I need for a 2 monitor set up as well as outputting to my TV via HDMI, also what about sound on the TV ?|||have to have a video card that will support 3 monitors, any radeon 5000 series or higher will do the trick if it is eye affinity compadable|||I have Ati HD 5750, it will do that.
